Instructions for Forms
Prior to the students arrival, the following below must be on file with the Resident Director. No student will be assigned a room until each of these forms are completed and returned. The forms are: Consent to Treat, Permission, Physical, Health Insurance
Several of these forms are available in digital form below and can be faxed to 314-993-4403 or sent through the mail to the attention of the Resident Director.
In addition to the forms listed below, the student should surrender his I-20 form and passport to the Resident Director as well when he arrives on campus.
